We didn't mention one of the oldest remote broadcast technologies.  It's using dedicated audio lines provided by the local telco.  Equalized lines were available, as well as non-equalized lines.  Some equipment used with such "nailed-up" lines (direct from the remote site to the studio) is shown here.
